Family dynamics related to cardiac rehabilitation

Patients recovering from CHD are usually encouraged to enter a period of cardiac rehabilitation. This rehabilitation process involves lifestyle changes that not only speed recovery but also help maintain cardiac health. Traditionally the focus of the cardiac rehabilitation process has been the recovering patient. Unfortunately this individual focus does not take into consideration the role the patient’s spouse or partner might play in the recovery process. Drawing on the Bowen Family Systems Theory, this study explores the couple dynamic in the cardiac recovery process by focusing on the constructs of differentiation of self and relationship triangles. Fifty-nine couples and four individuals agreed to participate in this study at Time 1 – within one week of their hospitalization. Participation in the study was limited to couples in a committed relationship whose male partner had been treated for CHD using angioplasty and/or cardiac stent. Participating couples were asked to complete a packet of self-report measures at Time 1. Three months later (Time 2) the participating couples were mailed a similar self-report packet of measures to complete. Patient and partner differentiation were positively related to their respective levels of marital satisfaction at Time 1. This did not prove to be the case for the partner at Time 2. Patients did make statistically significant changes in both diet and exercise from Time 1to Time 2. However, total differentiation was not significantly related to change in either diet or exercise for patient or partner. One component of patient differentiation at Time 1, “Fusion with Others,” was positively related to change in the patient’s diet. Patient’s perception of partner support was not related to partner level of differentiation. For patients whose partners made significant diet changes, there was a positive relationship between changes in the patient’s diet and the partner’s diet. Finally, there was no relationship between the patient’s level of differentiation and his entry into a formal cardiac rehabilitation program.
