Evaluating the Effectiveness of the Most Widely Used Intervention Strategies in Reducing 30-Day Hospital Readmission for CHF Patients Using AHP

In recent years, numerous intervention strategies have been introduced that allow reducing hospital readmission rate. This study evaluated and compared the impact of several intervention strategies that have been identified in past studies on thirty day heart failure hospital readmission rate. Five prior-At discharge strategies (Implement comprehensive discharge planning, Educate Patient/caregivers, Schedule and prepare for follow-up appointment, detailed discharge instructions, and Help patient manage medication), five post discharge intervention strategies (Promote patient self-management, Home Visit, Telephone, Tele-monitoring, and Outpatient (clinic) Visit) were used in this study. Systematic review was conducted in order to extract the most useful data from the studies that discussed the effectiveness of different intervention strategies on reducing 30 day readmission for CHF patients. These data were utilized to compare these intervention strategies in terms of their role in reducing thirty day readmission rate, their cost, simplicity, and dependency of each. Analytical Hierarchy Process (AHP) was identified and used in this study to systematically investigate and compare those strategies. About 30 studies were carefully analyzed to get the needed data for this research. The overall results showed that Educate patients/caregivers as prior-at discharge strategy and Home visit as post discharge strategy are the most recommended strategies, in other words, these strategies should be the first that come to minds when we look for the most effective interventions in reducing 30 day CHF hospital readmission rate with reasonable cost. It was also found that Readmission reduction followed by the cost had the most effect on selecting or ranking different intervention strategies.
