A Cost-Effectiveness Analysis of Methods of Screening for Dysphagia After Stroke

Dysphagia is common after stroke and contributes to subsequent morbidity and mortality. The objective of this study is to provide a cost-effectiveness analysis of dysphagia screening in the acute post-stroke period by videofluoroscopic swallowing study (VFSS), clinical bedside swallowing evaluation (CBSE), or a combined approach. A decision-analysis model was created using original database analysis and from published clinical studies. Model robustness was evaluated by univariate and probabilistic sensitivity analyses. The model assessed direct medical costs from a societal perspective. Strategies were compared on the basis of incremental cost-effectiveness analysis, with effectiveness measured in quality-adjusted life-years (QALYs). The VFSS appears to be the optimal strategy in identifying dysphagia, with a cost per QALY of $11,659 over CBSE, though limitations of model estimates prevent recommendation of this strategy.
