The Effect of Abacavir on Inflammation and Endothelial Cell Activation in Adults with HIV Infection

Abacavir has been associated with increased risk of myocardial infarction in HIV-infected adults. This retrospective cohort study was designed to determine the biologic mechanism. Stored plasma was used to compare changes in inflammatory and endothelial cell activation markers between antiretroviral-naïve patients initiating zidovudine, lamivudine and abacavir or the same regimen with a non-nucleoside reverse transcriptase inhibitor (NNRTI) and also zidovudine, lamivudine, abacavir and an NNRTI or the same regimen without abacavir. After adjustment, high sensitivity C-reactive protein decreased more in the group without the NNRTI, and soluble tumor necrosis factor receptor-II and soluble vascular cell adhesion molecule-1 decreased more in the group without the abacavir. When evaluating the association between abacavir and myocardial infarction, consideration should be made for whether an additional class of antiretrovirals is part of the abacavir-containing regimen. Further, inflammation and endothelial cell activation may play a role in the increased risk of myocardial infarction with abacavir.
