A Randomized Controlled Trial of Telemonitoring for Older Adults with Heart Failure Receiving Home Health Care

Home telemonitoring can augment home health care (HHC) services during a heart failure patient’s transition from hospital to home. HHC agencies in Ohio participated in a randomized trial of telemonitoring versus usual HHC. Rehospitalization, emergency department and urgent care visits were the combined outcome. Kansas City Cardiomyopathy Questionnaire (KCCQ) measured health status at admission and at discharge from HHC. Results showed that the study patients’ degree of comorbidity and heart failure severity were typical of a HHC population. There was no difference in rehospitalizations, emergency department and urgent care visits but an improvement in KCCQ scores for those who received telemonitoring versus usual care. The improvement in health status persisted with regression modeling. In conclusion, telemonitoring may not reduce rehospitalizations, emergency department or urgent care visits but health status is improved. For older adults with heart failure telemonitoring may be an important adjunct to HHC services to improve health status.
