Arthritis as First Presenting Symptom of Inflammatory Bowel Disease: A Case Control Study

Background: Inflammatory bowel disease (lBD) is frequently associated with arthralgia and arthritis. We hypothesized that differences exist in initial laboratory values and body measurements that can distinguish !BD associated arthritis from other forms of juvenile idiopathic arthritis (JlA) prior to the diagnoses of lBD. Methods: A retrospective case control study of children who presented with joint pain without overt gastrointestinal symptoms and who were subsequently diagnosed with IBD was performed. Age and gender matched controls were identified from children with JlA. Body morphometrics and laboratory findings were extracted by chart review. Results: 12 subjects and 24 controls were identified. Univariate analysis demonstrated that subjects with IBD associated arthritis had significantly lower weight, hemoglobin, albumin levels and higher CRP and ESR values. Multivariate analysis showed subjects with |BD-associated arthritis had elevated CRP and lower weight. Conclusion: These data suggest that in subjects presenting with arthritis, elevated CRP and lower weight percentile, IBD should be excluded causative.
