Outcomes of Deceased Donor Kidney Offers to Patients at the Top of the Waiting List

Transplant centers may accept or refuse deceased donor kidney offers to patients at the top of their waiting lists. The objective of this study was to determine the outcomes of deceased donor kidney offers (acceptance or refusal) based on characteristics of waitlisted patients and donors. The outcomes of all 7 million deceased donor kidney offers that occurred in the United States between 2007-2012 and resulted in transplantation were evaluated. Deceased donor kidneys were offered on average 7 times before being accepted for transplantation. The most common reason for refusal of a kidney offer was for donor-related factors or transplant center bypass (45% each). After adjustment, males and Hispanic patients were less likely to have an organ offer accepted compared to females and white patients. Transplant centers often refuse organ offers which may be associated with disparities for patients at the top of the waiting list.
