This study was designed to identify potential sociodemographic correlates and individual patterns of use for adolescent adherence to continuous positive airway pressure (CPAP) therapy. Poor adherence in adolescence has been well established in the obstructive sleep apnea (OSA) literature, yet to date, few interventions are available with demonstrated efficacy in addressing this issue. The current small sample analyses and case series were proposed to better understand the variables significantly associated with rates of adherence as well as individual usage patterns. Ten adolescent participants met inclusion criteria based upon examination of a pediatric sleep clinic database of CPAP users. Their usage was examined across three time points (end of first week of usage, end of first month of usage, and end of three months of usage) to investigate the possible relationships between sociodemographic and health variables with adherence rates and to offer perspectives on the patterns of adherence for adolescent users of CPAP. Overall, adolescent CPAP adherence was suboptimal, with only one participant in 10 using CPAP for 70% of nights, >4 hours per night, over a three-month period. Caucasian adolescents had better adherence to CPAP on all adherence variables as compared to African American adolescents at the end of the first week and at the end of the first month of use; however, this difference between groups was not maintained in the total percentage of nights used at the end of the first three months. When examined across all three time points, individual participants’ percentage of nights used suggests some trajectories that may help to inform cluster analyses of larger samples. Future interventions should fit with the developmental tasks of adolescence and also address racial/ethnic disparities in adherence to CPAP.
