Gender Differences in PTSD: An Exploration of Peritraumatic Factors

Epidemiological studies of posttraumatic stress disorder (PTSD) have consistently reported gender differences in the development of symptoms. Gender differences do not appear to be due to differences between men and women in types of trauma experienced, but rather, it has been hypothesized that gender differences in symptomatology may stem from differences in initial responses to trauma. The aim of the present study was to examine a number of initial reactions during the immediate aftermath of a motor vehicle accident (MVA) to determine whether they contributed to gender differences in PTSD symptoms. 357 adult MVA victims (210 males and 147 females) provided information on peritraumatic dissociation, mastery, perception of life threat and initial posttraumatic stress symptoms. PTSD symptoms were assessed at six-week and six-month follow-ups. Mediation models were conducted to determine whether the acute response variables accounted for gender differences in symptom development. Results suggested that only initial posttraumatic stress symptoms served as a significant mediator of the relationship between gender and PTSD symptoms. These results provide support for the hypothesis that initial responses to the trauma may contribute to gender differences in PTSD symptoms, but only with respect to initial PTSD symptoms.
