Development and Design of a Competency-Based Curriculum for an Integrated Community Psychiatry Nurse Practitioner Fellowship Program

The transition from a baccalaureate prepared registered nurse (RN) to a graduate advanced practice registered nurse (APRN), described as a shift from provider of care to prescriber of care, can be a difficult transition. Recently graduated APRNs report high levels of stress, anxiety, fear, frustration, feelings of inadequacy, low confidence, unclear expectations for the orientation period, and role isolation during their transition from graduate school into advanced nursing practice. APRNs commonly express feelings of being an “imposter” or “fake”. APRNs’ perceived competence and confidence increase over time in a supportive practice environment. There is a need for intentional and consistent mentorship and consultation during this critical first year of clinical practice that helps the recently graduated APRN absorb his/her new advanced practice professional role. In collaboration with the Robert Wood Johnson Foundation, the Institute of Medicine recommended the development of postgraduate education for APRNs. Fellowship programs can reduce nurse turnover, increase nurse job satisfaction, and improve clinical competence. The purpose of this DNP project was the development and implementation of a curriculum for an Integrated Community Psychiatry Nurse Practitioner Fellowship Program (ICPNPFP) designed for recently graduated PMHNPs to help improve competencies, self-confidence, and role socialization over the course of their first year of practice in an outpatient community mental health integrated care center. The fellowship program’s curriculum was designed according to program evaluation criteria that have been developed by the American Nurses Credentialing Center’s (ANCC) Practice Transition Accreditation Program (PTAP). This DNP project developed the core competencies, evaluation methods, and competency-based curriculum for the ICPNPFP.
