Comparing Robotic-Assisted and Laparoscopic Hysterectomy Conducted with and without Intravenous Acetaminophen

Anecdotally, clinicians and patients report less pain and quicker recovery following robotic-assisted hysterectomy (RAH) compared to laparoscopic hysterectomy (LAH). The primary purpose of this study was to examine patient outcomes between surgical routes while accounting for use of intra-operative intravenous acetaminophen (IV ACTM). A retrospective chart evaluation of n=248 patients who underwent RAH or LAH between January 1, 2010 and July 1, 2013 was conducted. Patients were matched by propensity scores for membership into four groups. The primary outcomes measured included opioid consumption, surgery time, estimated blood loss (EBL), and Post Anesthesia Care Unit length of stay (PACU LOS). A 2 x 2 factorial ANOVA was performed to compare the effects of RAH versus LAH conducted with and without IV ACTM. The combination of RAH and IV ACTM significantly reduced surgery time (p<0.05). Significant differences (p<0.001) were also found in EBL in patients who underwent RAH compared to patients who underwent LAH, regardless of IV ACTM administration. No statistically significant difference was found in morphine equivalents across groups. The combination of RAH and IV ACTM also significantly decreased (p<. 05) PACU LOS. There is evidence that the combination of RAH and IV ACTM reduced surgery time and PACU LOS, which merits further investigation of the relationship between RAH and IV ACTM.
