Assimilation Analyses of Cognitive Therapy for Generalized Anxiety Disorder: A Multiple Case Study

In this theory-building multiple case study, assimilation analysis was applied to the available audio records of four (3 successful, 1 unsuccessful) completed cases of cognitive therapy for generalized anxiety disorder (GAD). Four teams, consisting of the lead investigator and 2 co-investigators, reviewed each case independently. Teams observed the expression of major voices—the assimilation model’s name for the individual parts of a client’s personality—in the audio record of client’s in-session dialogue. The most relevant voices identified were labeled critic voices, and they were described by co-investigators as harsh and derisive aspects of the self associated with worry and self-doubt. Observations about critic voices made by each team were combined by the lead investigator, in consultation with his academic advisor, to construct rich narrative accounts of all four clients’ cases. In an unanticipated finding, critic voices did not directly produce anxiety through attack of other voices in clients’ communities. Rather, critic voices induced client vulnerability to specific, anxiety-arousing circumstances by derogating their coping skills or exaggerating the threat of external dangers. This tentative account of GAD was discussed in light of previous research with anxiety disorders and assimilation theory. Methodological issues, particularly the decision to use audio records to construct case conceptualizations, were also reviewed.
