Many collegiate athletes risk sustaining a sport-related mTBI. Identification of the initial injury and when the athlete has returned to previous levels of functioning is important in the management of mTBI. This prospective cohort study examined the frequencies of baseline testing, mTBI rates and neurocognitive and symptom recovery patterns of athletes competing in Division 1 sports at a single university over 19-years. Differences in athletes who returned to baseline on neurocognitive and symptom measures and those exhibiting persistent change was also examined. Athletes (n = 226) sustained 234 documented mTBIs. We presented all numeric descriptive statistics as means and standard deviations. Chi-square tests of association were used to determine if the data exhibited a significant first-recovery pattern in neurocognitive measures versus symptom recovery. We used linear regression to determine trends in mTBI rates. An increasing linear trend in mTBIs existed (p = .0014). Neurocognitive recovery took an average of 11.89 days and athletes took an average of 11.84 days to report being symptom free. Athletes demonstrating persistent change most commonly reported continued symptoms. MTBI management practices have improved. The wide variability in recovery and the evidence of persistent symptoms supports the use of a collaborative team management approach.
