The development and evaluation of a brief shame resilience intervention: Proof of concept in social anxiety disorder

The current project developed and tested a 2-session brief shame resilience intervention (BSRI) within individuals diagnosed with social anxiety disorder (SAD) and elevated levels of shame. Shame, or the experience of believing that one is flawed and unworthy of belonging, is implicated as a maintaining factor or potential mechanism across a range of mental health problems, including SAD. Despite theoretical and empirical evidence supporting the role of shame as a maintaining factor of SAD, and despite recommendations that shame be targeted in SAD interventions, to date, shame is not systematically incorporated into evidence-based treatments for SAD. The current study developed the BSRI and evaluated its perceived credibility, acceptability, and effectiveness. Additionally, using a non-concurrent multiple baseline design, we used a multi-method assessment to determine whether the BSRI led to reductions in shame-proneness, global and cue-related shame, and state shame in response to an in vivo stressor task. Further, we evaluated the effect of the brief shame intervention on SAD and depression symptoms and examined whether reductions in shame were related to reductions in social anxiety. Overall, findings demonstrated that the BSRI was highly acceptable to participants and that reductions in shame following the intervention were associated with reductions in SAD symptoms. However, findings also highlighted the need for further modifications to the BSRI given its more modest effect on reducing various indicators of shame and social anxiety.
