Effects of Bolus Consistency and Bolus Volume on Temporal Measurements of Pharyngeal Swallowing in Poststroke Patients

The purpose of this study is to examine the effects of bolus consistency and bolus volume on temporal measures of pharyngeal swallowing in poststroke patients. For this study, temporal measurements were obtained from 19 poststroke patients with aspiration (aspirators) and 33 poststroke patients with no aspiration (nonaspirators). The control group was comprised of 20 normal subjects undergoing Videofluoroscopic swallowing examinations (VFSEs), also from a previous investigation. The protocol was as follows: two swallows of 5ml thin liquid, two swallows of 10ml thin liquid, two swallows of 5ml nectar thick liquid, and two swallows of 10ml nectar thick liquid. The three temporal measurements of pharyngeal swallowing measured were Stage Transition Duration (STD), Laryngeal Closure Duration (LCD), and Duration of Upper Esophageal Opening (DUESO). Statistical comparisons were made by three-way analysis of variance (ANOVA) and the significance level was set at p<0.05. Bolus consistency and bolus volume did not have an effect on the three temporal measures of pharyngeal swallowing. However, the poststroke groups and the control groups differed significantly for the two temporal measures, STD and DUESO. Thus, among the three temporal measurements of pharyngeal swallowing, DUESO was the most sensitive in distinguishing between control subjects, poststroke patients with aspiration, and poststroke patients without aspiration.
