Treatment Compliance and Post-Treatment Behavior in Adolescents Attending Residential Treatment for Substance Use Disorders

Substance use in adolescents has significant negative long-term effects, and relapse post-treatment remains high. Despite the need for effective treatment programs, research on substance use treatment outcome has been too narrowly defined and there has been limited exploration of factors related to in-treatment behavior and premature treatment dropout, two factors, which may play an important role in treatment outcome. This naturalistic study aimed to identify predictors of an adolescent’s experience during and after residential substance use treatment. Analyses were conducted on 284 adolescents at intake and during residential substance use treatment and for 92 participants who completed the program and responded to follow-up questionnaires one month after discharge. Bivariate analyses and a binary logistic regression analysis were conducted to determine whether variables representing five domains (sociodemographics, substance use, legal involvement, psychological functioning, and social relations) significantly predicted treatment completion. Legal history and no assessed risk of harm to self at intake were protective factors against premature treatment dropout. Further exploration of treatment compliance revealed significant concerns with the existing structure of the SASSI-A2 based on confirmatory factor analysis. Structural equation modeling revealed no significant relationship between substance use severity and treatment compliance, and motivation to change did not mediate this relationship. Finally, logistic analyses were conducted to explore a more robust examination of post-treatment outcome. Significant relationships between relapse post-treatment and AA/NA attendance, individual psychotherapy, group counseling, legal problems, and the perception of having a very supportive mother were found.
