An Investigation of the Impact of Client Requirements for Alliance on the Alliance-Outcome Association

Psychotherapy theory and research has suggested that clients may vary in the extent to which they require certain therapy processes or therapeutic interactions to occur in order to improve. Although these client requirements might moderate the relationship between psychotherapy processes and outcomes, they are rarely studied in the treatment literature. The current research investigated the relationships between client requirements, the strength of the working alliance/therapeutic bond, and therapy outcomes. Participants were adult clients presenting for treatment at a psychology department clinic (N = 70). Clients completed a novel measure of client requirements for working alliance at intake, the Working Alliance Inventory-Short Revised- Client Requirements. Working alliance, symptoms, and session quality were measured at each session. Client requirements for alliance/bond was not found to be a significant moderator of the alliance/bond – outcome relationship. This may suggest that clients do not know what they need from therapy, or that client needs do not impact the alliance-outcome association. However, there were several study limitations that may have led to commission of Type II error.
