Comparison of Diagnostic Interviews for Children Accessing Outpatient Mental Health Services

Evidence-based assessment is important to the field of childhood psychopathology (Mash & Hunsley, 2005). Researchers and clinicians frequently use structured diagnostic interviews to ensure reliability. One commonly used diagnostic interview is the Schedule for Affective Disorders and Schizophrenia for School Aged Children (K-SADS; Kaufman et al., 1997; Puig-Antich & Ryan, 1986). The K-SADS is semi-structured and assigns DSM-IV diagnoses from parent and child report. Despite frequent research use, the K-SADS is too long for most clinical uses. The Children’s Interview for Psychiatric Syndromes, (ChIPS/P-ChIPS; Weller, Weller, Rooney, & Fristad, 1999a; 1999b) is a highly-structured diagnostic interview that also utilizes DSM-IV criteria. Brief enough for clinical settings, the ChIPS and P-ChIPS are less commonly used in research. The present study compared K-SADS and ChIPS/P-ChIPS diagnoses in an outpatient clinical sample. Fifty parent-child pairs (age 7-14) were recruited from a larger study. Agreement between K-SADS and ChIPS/P-ChIPS diagnoses was moderate to high for most diagnoses. K-SADS and ChIPS interviews also showed high rates of agreement with consensus diagnoses assigned by a clinician. ChIPS interviews were significantly shorter than K-SADS interviews, but the difference was less than expected based on previous studies. Interviewer experience level and participant demographics did not appear to affect agreement. This study’s results are further evidence for the validity of the ChIPS and support for its use in clinical and research settings.
