Abstract
Frailty is a complex condition without specific definition. It has been described in various models as an accumulation of losses, disabilities, and/or symptoms. Without a specific definition, its true prevalence is unknown. The primary purpose of this study was to compare three models of frailty currently in use (Rockwood, Strawbridge, and Fried and Walston). These models were compared on: prevalence, sex-specific prevalence, and the association of model-defined frailty with biomarkers of frailty. Bivariate and multivariate analyses were used in this cross-sectional analysis of NHANES III data to examine within and between model characteristics. Rockwood and Fried and Walston Models showed the greatest agreement in assessing an individual’s current frailty state. Females had a significantly higher prevalence of frailty in each of the three models examined. Hemoglobin, C-reactive protein, and Vitamin E were associated with frailty overall. Vitamin E had a significantly increased likelihood of abnormality related to frailty in males, with hemoglobin, C-reactive protein and Vitamin A significant in females. This study suggests the Rockwood and Fried and Walston models of frailty are capturing similar states, showing prevalences between 8-10%. Biomarkers of aging do differ between males and females, lending support to the idea that frailty is expressed differently in males and females.
