The Influence of State and Trait Perseverative Cognition on Autonomic and Hemodynamic Regulation

Given the well-known disparity in hypertension (HTN) incidence and prevalence between African Americans and Whites and prospective evidence for the contributing role of negative psychological factors to HTN risk, contemporary research has begun to evaluate how such factors contribute to the development of pathologic BP. The Perseverative Cognition hypothesis (PC), builds on this approach by suggesting that the process of worrisome or perseverative thinking represents a significant disease pathway through chronic or repeated cognitive representations of various stressors. Importantly, the link between PC and CVD risk has been explicitly described in the context of a neurovisceral pathway with direct and indirect links to the regulation of BP and the heart. In previous research (Hill, 2009), a sample of young, healthy, African Americans (AA) identified as having high trait PC exhibited elevated sympathetic nervous system-mediated hemodynamic activity and greater cardiac parasympathetic control (HRV). This pattern represents a `cardiovascular conundrum’ whereby elevations are observed for both a risk and a potential protective factor for cardiovascular disease. The present research extends this previous work by demonstrating that state PC is associated with a unique pattern of hemodynamic regulatory activity in both healthy European American and AA men and women.
