Abstract
Statement of Problem: Internal conical implant-abutment connections without horizontal platforms may lead to axial and horizontal displacements of crowns during screw tightening and torquing. These displacements may affect the proximal contacts, incisal edge position, or occlusion. Purpose: To evaluate whether there is a displacement of custom screw-retained zirconia single crowns (Atlantis, Dentsply) into a recently introduced Dentsply - Astra EV internal conical seal implant-abutment connection in three dimensions during screw tightening by hand and via torque driver and compare the amount of crown displacement with the previous implant internal connection. Materials and methods: Stereolithic acrylic resin models were printed using computed tomography data from a patient missing the maxillary right central incisor. Two different internal connection implant systems (OsseoSpeed, Dentsply -Astra Tech 4.0S- Å ~11.5-mm and EV implant system, Dentsply -Astra Tech 4.2S- Å~11.5-mm) were placed in the edentulous site in each model using a surgical guide. Five screw-retained zirconia milled single crowns (Atlantis, Dentsply) were fabricated for each system. Proximal contacts were adjusted to allow an 8 µm tin foil shim to drag between interproximal contacts without tearing. Incisal edge positions were adjusted to coincide with the adjacent central incisor. The crowns were then subsequently tried on the stereolithic model, which represented the patient. The relationship of the crown to the patient model was recorded using a pair of high-resolution digital cameras. The crowns were tightened to 20 N cm for OsseoSpeed implant and 25 N cm for EV implant using a torque driver, and the cameras recorded their relative position again. Three-dimensional image correlation was used to measure and compare crown positions: first hand tightened, then torque driven. The displacement test was repeated 3 times for each crown (times 1, 2, and 3). Commercial image correlation software (Vic-3D) was used to analyze and extract the data to find the amounts of displacement vertically, mesiodistally and buccolingually to the EV implant and compare these values to the OsseoSpeed implant. A repeated-measure ANOVA was used to calculate the mean displacements for each of the 10 Atlantis crowns. A t-test with Bonferroni correction was used for pairwise comparison of each crown displacement to determine statistical differences (a=.05). Results: The mean vertical displacements (10.5 µm for OsseoSpeed and 13.2 µm for EV implant) were statistically higher than mean displacements in the M-D (1.2-2.5 µm) and B-L (2.2-6 µm) directions. Mean displacements in all directions were statistically significant between times 1 and 2, and times 1 and 3 for both implants. No statistically significant difference was found for displacements between implants (OsseoSpeed or EV implant) at different directions and at different times (P > .05). Conclusions: Within the limitations of this study, screw-retained zirconia crowns fabricated on OsseoSpeed and EV implants had the tendency to displace in all 3 directions (V, M-D, B-L) with highest mean displacement in the vertical direction at test time 1. However, the amount of displacement of crowns between these two different implant systems was statistically insignificant at all directions and times.
