Identification of Malnutrition in the Gastroenterology, Hepatology, and Nutrition Outpatient Clinics Using the Malnutrition Screening Tool

Individuals with gastroenterology diagnoses are at risk for malnutrition due to the complications associated with their conditions. The potential to impair adequacy of oral intake, digestion and absorption exists within many of the most common diagnoses seen in an outpatient gastroenterology clinic. However, many of these individuals do not receive routine nutrition care from Registered Dietitian Nutritionists in the outpatient setting and medical nutrition therapy for these diagnoses is not routinely reimbursed by insurance. This study attempted to identify the percentage of patients who were at malnutrition risk and categorize nutrition risk by diagnosis category. A cross-sectional, retrospective medical record review was conducted for a convenience sample (N=559) of patients receiving medical care at outpatient gastroenterology, hepatology, and nutrition clinics. Malnutrition risk was assessed using the Malnutrition Screening Tool (MST). Data was stratified according to risk of malnutrition and diagnosis category: gastrointestinal (GI), hepatic, or pancreatic. Patient anthropometrics and GI symptoms were assessed. A majority of the patients had a diagnosis of GI origin (77%), 50% had a hepatic origin and 18% had a diagnosis of pancreatic origin. Of the 559 patients, 17.9% were classified as at-risk for malnutrition, with the greatest risk in patients with a pancreatic diagnosis. The use of the MST identified a large percentage of patients at potential nutritional risk and used very few clinical resources in doing so. Raising awareness for malnutrition risk will allow for appropriate referrals and initiate early interventions for at-risk populations.
