Differences in Human Heart Rate Variability Due to Trait and State Worry: A Meta-Analysis

The present meta-analysis aimed to assess current findings on the association between worry and autonomic functioning, indexed by measures of heart rate variability (HRV) in human subjects. An extensive and inclusive literature search yielded 2312 titles and abstracts, of which 24 provided sufficient reporting of worry and HRV characteristics. Data from 1230 participants were available for analysis. Meta-analysis resulted in effect size estimates across 4 different measures of state and trait worry and 6 measures of HRV. Individuals clinically diagnosed with Generalized Anxiety Disorder (GAD) showed a significantly lower mean HRV than healthy controls. In addition, actively worrying individuals showed significantly reduced levels of HRV on average when compared to at rest. When combining individuals from both clinical and non-clinical high trait worry groups, trait worry was shown to be negatively associated with HRV. Meta-regression showed that this was not significantly affected by age, sex, clinical diagnosis, or worry severity. While high trait worry was shown to be associated with lower levels of HRV on average when compared to controls, the magnitude of the effect illustrated ample overlap between these groups such that individuals with high trait worry may potentially possess similar and even higher levels of HRV than a portion of controls.
