Differences in Distribution of Carbohydrate across the Day by Levels of Glycemic Control in US Adults

Objectives: To analyze carbohydrate intake by distribution across the day of the US adult population based on level of glycemic control. Methods: In this retrospective analysis, NHANES data collected from 2005 – 2014 was analyzed. US adults (n=20,034) over the age of 30 were sorted by level of glycemic control: Normal Glycemic (NG) HbA1c<5.7%, Prediabetes (PRE) HbA1c 5.7-6.4%, Controlled Diabetes (CD) HbA1c 6.5-6.9%, Poorly Controlled Diabetes HbA1c 7.0%. Dietary intakes were analyzed for average totals and distribution by time of day for energy, carbohydrate, sugar, fiber and complex carbohydrate. Results: There were minimal differences in carbohydrate distribution between all sample groups. Approximately 60% of each group consumed a low intake (<25% of the day’s total) of carbohydrate in the morning and a high intake (35% of day’s total) of carbohydrate intake in the evening with approximately 40% of all carbohydrate consumption occurring in the evening. Conclusions: A consistent carbohydrate distribution is preferred to help reduce high postprandial glucose peaks in individuals with type 2 Diabetes Mellitus. However, regardless of level of glycemic control, the majority of the US adult population in this sample consumed an uneven distribution of carbohydrate with lower intake in the morning and higher intake in the evening. A consistent carbohydrate distribution is beneficial for individuals with type 2 diabetes. Emphasis of a consistent intake during diabetes education may be beneficial for individuals struggling with glycemic control.
