Cascade testing communication within Lynch syndrome families: An examination of communication privacy management theory

This thesis used Communication Privacy Management Theory (CPM; Petronio, 2002) to explore how Lynch syndrome (LS) patients disclose and discuss their genetic testing results with at-risk family members, a process referred to as cascade testing. Qualitative, semi-structured interviews were conducted with colorectal cancer (n = 9) or endometrial cancer (n = 5) patients with LS. Interview transcripts were coded and analyzed with the application of CPM’s main theoretical components (privacy ownership, privacy control, and privacy turbulence) and predictions (e.g. axioms). While the data provide support for several of the CPM axioms, others were not supported. Perhaps most importantly, we found that participants did not view LS information as `private’ information they were the `sole owners’ of – a finding counter to a key prediction of CPM, Axiom #1. Rather, LS information was readily disclosed to families and open communication about LS was encouraged by participants. The results of the present study provide novel findings for family health communication in the cascade testing context. The theoretical implications for CPM along with practical applications for genetics health professionals are discussed.
