The Fate of Untreated Primary Second Molars Under General Anesthesia

Objectives: This investigation looked at new and recurrent caries patterns, need for second dental GA and demographic characteristics of young children treated under GA. Methods: A retrospective chart review was conducted for a randomly chosen subset of all ASA 1 or 2 status children,24-48 months of age, treated under GA who returned for follow up within 18 months. Data included age at GA, gender, teeth treated, language, treatment decisions for second primary molars, new caries experience on primary second molars and need for second dental GA. Results: Of 2018 (53% male) subjects treated under GA, 763 (37.8%) returned for Hygiene visit within 6-18 months following GA. Median age at GA was 39 months. Median number of teeth treated was 12. The majority of families (63%) spoke English; the remainder spoke a variety of languages. In 48% of patients, all primary second molars were covered with stainless steel crowns (SSC) at GA. Of this subgroup 4% had second GA. In 39.7% of patients at least one second molar did not receive SSC at GA. Of this subgroup, 27% and 50% had at least one carious primary second molar by 18 months and 60 months post-GA respectively. Of this subgroup, 11.5% had second GA. In 12% of the patients, the primary second molars were not fully erupted at the GA. Of this population 48% and 63% had at least one carious primary second molar at 18 and 60 months post-GA respectively. Of this subgroup, 41% had second GA. Statistically significant associations were found between higher odds of second GA and not covering all primary second molars at GA, non-erupted primary second molars at GA, lower age, higher number of treated teeth at GA and English language. Conclusions: Many young children, treated under GA maintain high caries risk post-GA. Non-carious primary second molars present at GA had a significant risk of new caries. Considering GA risk and cost, postponing GA until full eruption of second molars, and taking a more aggressive approach to treating primary second molars under GA, may reduce likelihood of subsequent GAs.
