Assessing the Lived Experience of the Family Nurse Practitioner in Urgent Care Practice

Family Nurse Practitioners (FNPs) are playing a bigger role in the care of emergency/urgent patients every year. FNPs lack educational opportunities to gain experience with the essential knowledge, skills and behaviors needed for a nurse practitioner to be successful in the Urgent Care (UC) practice environment. Research on skill acquisition and competencies for FNPs working in UC is limited. Foundational work in this area by the Emergency Nurses Association (ENA) was utilized as the basis for this project. Benner’s Novice to Expert skill acquisition model was utilized as the theoretical framework. A qualitative, descriptive research study was designed using focus group data collection. Appreciative Inquiry (AI) helped guide the focus group discussion. Analysis of the focus group data allowed seven common themes to emerge, which were collectively representative of the participants’ transitions into the UC practice environment. The seven themes identified were: quality and consistency of preceptors, role definition, serving many masters, time, see- do- teach, survival guides, and expert back to novice. FNPs working in an UC setting identified unique issues associated with the role transition that are important factors in planning and implementing FNP learning opportunities. By understanding the lived experience of the FNP in the UC setting, additional educational opportunities can be developed in the form of a fellowship or nurse residency to increase the self-efficacy, self-confidence and knowledge of FNPs working in this specialized area.
