Maternal Diabetes Mellitus and Neonatal Hearing: A Retrospective Study of Hypoxic/Ischemic Related Risk Factors

Objective: The objectives of this study were (1) to examine the occurrence of hypoxic/ischemic related risk factors in infants of diabetic (DM) mothers and (2) to determine whether these infants are at greater risk for hearing loss than infants of non-DM mothers. Methods: Retrospective research data collection was conducted on approximately 100 DM mother and infant medical records and audiology files. Data tables were constructed in order to compare the incidence of variables associated with hearing loss in the DM population. The following variables were considered to be related to hypoxia/ischemia and significant to my research: asphyxia, respiratory distress syndrome, cerebral edema, low Apgar scores, macrosomia or high-birth weight, positive pressure resuscitation (PPR), pH below 7.4, seizure, muscle tone disturbance, intubation and/or assisted ventilation, irregular heart rate or rhythm, and other abnormal fetal signs such as meconium aspiration and placental insufficiency. Results: Compared to incidence data reported by Vohr et al. (2000), there were 6.3% more high-birth weight infants of DM mothers in the study population. Hearing screening outcome failure rates in the infants of DM mothers study population indicated 10.6% for OAE and 20.0% for ABR in the well-baby nursery, with and without hearing risk factors. The failure rate was 14.3% for OAE and 0.0% for ABR in the NICU population. There were very few infants of DM mothers with the hypoxic/ischemic related variables that failed on any hearing screening test.
