Genetic Epidemiology of Radiation Sensitivity and Basal Cell Carcinoma in Childhood Cancer Survivors

Purpose: Basal cell carcinoma of the skin (BCC) is the most common subsequent malignant neoplasm among childhood cancer survivors. Ionizing radiation is an established risk factor in this population. We assessed the impact of genetic sensitivity to radiotherapy as an additional risk factor for BCC. Patients and Methods: Participants in the Childhood Cancer Survivor Study (CCSS), who were treated with radiation therapy and who provided biospecimens, were included in the study. Subsequent BCC cases (n=321) and controls without BCC (n=825) were screened for 128 SNPs in radiation-related DNA repair genes. Results: The most important risk factors for BCC were radiation dose, age at original cancer diagnosis, and time at risk (p<0.001). An original diagnosis of leukemia was also highly significant (p=0.005). We were unable to identify significant SNPs related to radiation sensitivity. Conclusion: No significant SNPs were identified with an increased risk for BCC. Known risk factors (radiation dose and age at diagnosis of original cancer) were associated with an increased risk, suggesting that genetic susceptibility to radiation likely plays a minor role, if any, for increasing BCC risk.
