Indexing Left Ventricular Heart Mass in Children: Age Specific Reference Intervals

A previously analyzed dataset containing data used to create left ventricular heart mass measures from normal echocardiographic studies of 2247 children and adolescents was used as a basis for a study expanding the existing state of the art for producing age specific reference intervals. The study explored and compared the currently available statistical methods, parametric and non-parametric, to model the relationship of LV mass to body size, specifically height, in children and adolescents, ages 0 through 18 years. Allometric powers of height to index LV mass under a number of scenarios were determined. Using the most prevalent methods, models were created and compared. Simulation study analyses were carried out to help make the determination of which methods and models were most robust and efficient. The methods examined in this study included quantile regression, and extensions of the LMS method, which is used for constructing normalized growth standards, and which monitors the changing skewness of the distribution of the response variable during childhood. Also, a non age-specific technique commonly employed in studies of heart dimensions was used. The best methods were used to calculate age and gender specific centile and z-score curves and nomograms, and centile and z-score tables for indexed LV mass.
