Maternal Obesity is an Independent Risk Factor for ICU Admission during Hospitalization for Delivery

Objective: We aim to quantify the impact of obesity on peripartum maternal ICU admission, as recorded on the Ohio birth certificate. Methods: This is a population-based, retrospective cohort study of Ohio live births from 2006-2012. The primary outcome is ICU admission. The primary exposure is maternal body mass index (BMI in kg/m2), categorized by BMI < 18.5, BMI 18.5-24.9, BMI 25-29.9, BMI 30-39.9, BMI 40-49.9 and BMI = 50. We calculated the relative risk of maternal ICU admission based on BMI category. Multivariate logistic regression quantified the risk of obesity on maternal ICU admission after adjustment for hypertension, diabetes, Medicaid insurance, and preterm delivery. Results: This study includes 999,437 births, with peripartum maternal ICU admission rate of 1.10 per 1000. The rate of ICU admission for women with BMI 30-39.9 was 1.24 per 1000, relative risk (RR) 1.20 (95% CI 1.07, 1.35), BMI 40-49.9 had ICU admission rate of 1.80 per 1000, RR 1.73 (CI 1.38, 2.17), and BMI = 50 demonstrated ICU admission of 2.98 per 1000, RR 1.73 (CI 1.77, 4.68). These risk increases persisted in women with BMI 40 to 49.9 with adjusted relative risk (adjRR) of 1.37 (95% CI 1.05, 1.78) and women with BMI = 50, adjRR 1.69 (CI 1.01, 2.83), after adjustment for co-existing risk factors. Conclusion: Morbid obesity is a risk factor for ICU admission, with risk increasing as BMI increases. After adjustment for co-existing risk factors, BMI 40 to 49.9 and BMI = 50 are independent risk factors for maternal ICU admission.
