Obstructive Sleep Apnea and the Challenges of Positive Airway Pressure Therapy: Designing for Chronic Disease Management

A team of engineers at the University of Cincinnati College of Medicine are developing a new technology for Positive Airway Pressure Therapy in the treatment of Obstructive Sleep Apnea. This technology and the latent user needs that have not been met by current solutions in the market, create an opportunity for design. This work is divided intro three phases: Understanding, Making and Consolidating. Literature review provided background information on the prevalence and morbidity of the disease, the challenges of therapy and the lack of adherence. During the understanding phase more than 50 in depth face-to-face interviews were conducted, which provided a broad comprehension of the user experience with treatment and the PAP device, the commercial environment and the stakeholders involved. A set of criteria was defined from the learnings and applied during the making phase into were ideation, prototyping and user testing of headgear concepts with users. The consolidation phase comprehends the discussion and conclusions of the thesis. The insights from the second phase were summarized into a list of recommendations for future design research. In conclusion, the problem of adherence is highly complex, which makes it necessary to consider all stakeholders for a solution to be successful. The role of the designer in medical design can make the difference in getting a well-engineered product to produce good clinical outcomes, because the emotional dimension of therapy is critical in the management of chronic disease.
