The Role of Nutrition Assessment in the Indication of Gastrointestinal Complications in Adults Undergoing Allogeneic Hematopoietic Stem Cell Transplantation: A Case Report

Allogeneic hematopoietic stem cell transplantation (HSCT) is a fundamental component in the treatment of several hematological malignancies and disorders. The underlying disease and preparative regimen mediate an inflammatory response that is further induced by the development of acute graft versus host disease (GvHD). Inflammation of the gastrointestinal (GI) tract leads to complications including mucositis and diarrhea, resulting in insufficient oral intake and malabsorption that significantly influences the nutritional status of adult patients. Nutrition support plays a crucial role in addressing the GI complications observed during the acute period (i.e. admission to 100 days post-HSCT). However, the factors influencing the development of GI symptoms and the relationships through which these factors influence GI symptom incidence and severity in allogeneic HSCT are poorly understood. The purpose of these case reports is to share the investigation of the physical, biochemical, and nutritional factors observed in accordance with the development of GI complications and exploration of optimal methods for assessing nutritional status in adults undergoing allogeneic HSCT over four time points (i.e. admission, day +0, day +7, and day +21) during the acute period. Four patients were enrolled from inpatient and outpatient units of the University of Cincinnati Medical Center (UCMC) and the Jewish Hospital – Mercy Health. Questionnaires on demographics, nutritional factors, and GI symptoms were completed and blood sample analyses for inflammatory biomarkers were collected at admission, on day of transplant (i.e. day +0), and 21 days post-HSCT. The patients consumed diverse dietary patterns, had varying nutritional statuses and degrees of inflammation, and experienced similar GI complications. This study emphasized that the changes in the nutritional intake of adults undergoing allogeneic HSCT should be closely examined proportionately with GI symptom experience, which may aid in establishing an optimal assessment of nutritional status and improve transplant outcomes and quality of life.
