THE NEUROPSYCHOLOGICAL CHARACTERIZATION OF CHILDREN OF PARENTS WITH BIPOLAR DISORDER

The purpose of the present study was to investigate the pattern of neuropsychological strengths and weaknesses among children genetically at risk for bipolar disorder in an attempt to better understand early cognitive impairments associated with the illness. The neuropsychological functioning of children (ages 8-12) who had at least one biological parent with bipolar disorder (N=21) was compared to performance in a control group of children of healthy parents (N=20) matched for age, sex, socioeconomic status and race. The results indicate that at-risk children who met DSM-IV criteria for a mood disorder (N=11) as well as at-risk children who were not mood syndromal but had subsyndromal symptoms or were asymptomatic (N=10), performed more poorly on measures of executive functioning and were more likely to demonstrate a profile of a significant VIQ>PIQ discrepancy than control children. In addition, at-risk children with abnormal mood syndromes performed more poorly on measures of academic achievement than did non-syndromal at-risk children and healthy controls. These results extend the current literature on neuropsychological functioning of children at risk for bipolar disorder and suggest that both mood syndromal and non-syndromal children show specific cognitive impairments. The findings are consistent in part with the neuropsychological literature on adults with bipolar disorder and are most suggestive of primary executive dysfunction, rather than a right hemisphere impairment or nonverbal learning disability as previous literature has suggested. Executive impairments may be associated with the development and presentation of mood disorders in these children.
