Use of a Shared Decision-Making Aid in Pediatric Autism Spectrum Disorder

The purpose of this DNP project was to increase the use of a shared decision-making aid by providers and families of children with Autism Spectrum Disorder to guide the management of challenging behavior. Shared decision-making is a process of communication in which the family identifies priorities in the context of the child's condition and daily care, the healthcare team shares evidence-based care treatment options and clinical expertise and together decide on an optimal plan of care. Shared-decision making aids are written, visual guides comparing treatment options, risks, and benefits. A growing body of evidence reveals improved quality of life outcomes in children with developmental disorders when shared decision making is practiced and supported using an SDM aid. A quality improvement methodology was used to establish aims, define measures and identify evidence-based interventions to test changes in an outpatient environment specializing in the care of the pediatric ASD population. An interprofessional team conducted iterative tests of change which resulted in improvement of identification of the parent priority from a baseline median of 72 % to 85%. Efforts to shift the paradigm of healthcare delivery for children with ASD and integrate SDM with evidence-based practice may provide the needed decision support to optimize self-management and long-term quality of life outcomes for this growing population.
