Caregiver Involvement, Psychiatric Services, Caregiver Stress, and Treatment Outcomes for Maltreated Preschoolers

Children who are victims of maltreatment have significant psychological problems compared to children in the general population. Therefore, it is important for maltreated children to receive early and effective mental health treatment. The purpose of this study was to evaluate the Therapeutic Interagency Preschool (TIP), a program for victims of maltreatment, and examine factors that may contribute to treatment gains for 60 two to six-year-old preschoolers at TIP. More specifically, this study examined the association between caregiver involvement in treatment, psychiatric consultation, caregiver stress, and treatment outcomes. Results suggest that caregiver involvement in treatment is associated with significant improvements in externalizing behaviors in children and reductions in caregiver-reported stress. There were no changes in treatment outcomes or caregiver stress as a result of psychiatric consultation, and children involved in psychiatric services were no less likely to have caregiver involvement than children without psychiatric services. Finally, caregiver stress did not predict treatment outcomes. To improve outcomes for maltreated children, early intervention programs should consider the impact of increased caregiver involvement in treatment. Although previous studies have shown the benefit of caregiver involvement in children’s mental health treatment, this is the first study to do so with a sample of maltreated children whose caregivers are often foster parents rather than biological parents.
