Predictors of Dropout in Clients Treated for Obsessive Compulsive Disorder with Exposure and Response Prevention

Exposure and response prevention (ERP) is considered the gold-standard treatment for OCD (Amir, Kuckertz, Naimi, & Conley, 2015; Hebbar, 2013; Olatunji, Cisler, & Deacon, 2010; Ong, Clyde, Bluett, Levin, & Twohig, 2016). However, many clients find ERP too challenging or uncomfortable, as it requires facing intentionally anxiety-provoking stimuli and restraining from engaging in anxiety-reducing behaviors (Strauss et al., 2015), which likely contributes to the 25-30% dropout rate in individuals receiving ERP treatment for OCD (Abramowitz, 2006). In this study, anxiety sensitivity, demographic characteristics (age, gender, employment status, and marital status), and common comorbid diagnoses (major depressive disorder, generalized anxiety disorder, and panic disorder without agoraphobia) were examined as possible predictors of premature termination from ERP treatment for OCD using binary logistic regression models. Participants consisted of 284 adults who attended a partial hospitalization program (PHP) for OCD and other anxiety disorders at one of 10 behavioral health center locations across the country. Forty-three (15.1%) participants dropped-out prior to completion of the seventh treatment session and 96 (33.8%) dropped out prior to completion of the 12th treatment session. Findings suggest that neither anxiety sensitivity (as measured by the Anxiety Sensitivity Index; Reiss, Epstein, & Gursky, 1986), demographic characteristics, nor common comorbid diagnoses found in individuals with OCD predicted premature termination (when defined by termination prior to completion of the seventh treatment session and the 12th treatment session). Future research needed to better determine predictors of premature termination from ERP treatment for OCD are addressed.
