Ultrasound and exercise in skeletal muscle regeneration

Objective: To determine whether non-thermal therapeutic ultrasound (US) and low-intensity exercise (Ex) influence skeletal muscle regeneration following a standardized contusion injury in an animal model. Hypothesis: Compared with the three other contusion injured treatment groups, which received exercise treatment alone (Ex+NoUS), ultrasound treatment alone (NoEx+US), and no treatment (NoEx+NoUS), the Ex+US group will exhibit an increase in measures of skeletal muscle regeneration. Design: Randomized control trial with blinded comparisons in a 2 X 2 factorial (ultrasound X exercise) design. Setting: Animal care facility/exercise physiology biochemistry laboratory. Participants: Twenty male Wistar rats (8 mo.) received a reproducible bilateral contusion injury to the gastrocnemius (GTN) muscles. Interventions: Ultrasound (parameters = 3 MHz, 0.1 W/cm2, 1cm2 transducer, 5 min duration/day) and exercise (20 min/day of low-intensity treadmill walking at 14 m/min). Main Outcome Measures: 96 hour post-injury muscle mass, contractile protein concentration, fiber cross-sectional area (CSAf), number of nuclei per fiber, and myonuclear density. Results: There was a statistical difference (p<0.05) between injured and non-injured GTN muscles in terms of myonuclei per fiber. There were no statistical differences (p<0.01) between the 4 treatment groups for any of the outcome measures chosen as biomarkers of skeletal muscle regeneration. Conclusions: The specific hypothesis was not supported. There is no evidence that the specific continuous ultrasound and exercise protocols investigated interact to enhance skeletal muscle regeneration following contusion injury.
