IBUPROFEN/ACETAMINOPHEN VERSUS SPRIX IN TEETH DIAGNOSED WITH PULPAL NECROSIS AND SYMPTOMATIC APICAL PERIODONTITIS

Introduction: There is an increasing need to find alternative analgesic medications in dentistry. Currently the combination of ibuprofen/acetaminophen is recommended as the standard for pain control in endodontics. This is based mostly from studies that used an oral surgery pain model. Sprix is an alternative analgesic that can be delivered intransally. The purpose of this study was to compare the analgesic efficacy of ibuprofen/acetaminophen to Sprix in patients with untreated symptomatic pulpal necrosis. Materials and Methods: Thirty-five subjects in moderate to severe pain with a diagnosis of pulpal necrosis received either a combination of 600 mg ibuprofen/1000 mg acetaminophen and a placebo nose spray or 31.5 mg Sprix and placebo oral medication. Subjects rated their pain on a 170 mm Heft-Parker Visual Analogue Scale every 15 minutes for a total of 4 hours. Results: Both groups had a reduction in pain. Maximum pain relief was achieved at the 3 hour mark for the ibuprofen/acetaminophen group and at the 4 hour mark for the Sprix group. Mean pain for both groups at the conclusion of the study was in the mild category. There was no statistically significant difference in pain relief between groups at any time point in the study. Summary and Conclusions: Sprix was not significantly different than the combination of 600 mg ibuprofen/1000 mg acetaminophen in reducing pain in untreated symptomatic patients with a diagnosis of pulpal necrosis up to 4 hours.
