Abstract
An ascending aortic aneurysm is an asymptomatic enlargement of the initial segment in the thoracic aorta that has been described for almost two thousand years. This disorder can be fatal without medical intervention due to rupture or dissection leading to retrograde blood flow into the fibrous heart sac causing cardiac tamponade. Currently, the only effective treatment option is surgical intervention because there is no known medical therapies. NOTCH1 was initially linked to congenital heart disease, specifically aortic valve disease. Bicuspid aortic valve has a well-known association with ascending aortic aneurysms and was seen in a subset of patients with a mutation in NOTCH1. In 2017, Koenig, et al. described a murine model of ascending aortic aneurysms caused by haploinsufficiency of Notch1. The goal of this study is to test the current pharmacologic treatment options (propranolol and losartan) on ascending aortic aneurysms and characterize the response of Notch1+/- ascending aortic aneurysms to these different treatments. This study shows that the murine model of Notch1+/- develops saccular ascending aortic aneurysms independent of bicuspid aortic valve. It also shows that losartan protects against the development of aneurysms by echocardiography, gross analysis, and histology. As the aneurysms are confined to a specific location on the ascending aorta in Notch1+/- mice, the results of the study suggest an important role for hemodynamics in aneurysm development and progression.
