Pediatric Hepatology Training Needs Assessment

Introduction Pediatric gastroenterologists are expected to be experts in the diagnostic and clinical management of diseases that affect the entire gastrointestinal organ system, this medical knowledge and expertise are attained during a 3-year fellowship training. Given the estimated increase of liver disease and predicted pediatric hepatology provider shortage over the upcoming decade, pediatric gastroenterologists need to be comfortable with management of liver pathologies. 8 There are no studies to assess whether the PGI fellowship curriculum prepares specialists for children with liver diseases Methods A survey-based hepatology education needs assessment was disseminated electronically to the PGI fellows training at institutions with the United States during the academic year of the 2021-2022. Survey responses were collected via RedCap, a secure data collection website and standard descriptive statistics was used to analyze responses. Results A total of 134 (40%) fellows responded from 56 training centers across the United States. The results of this survey did demonstrate differences between training programs in terms of clinical experience. 81% of respondents training at liver centers of variable volume, admissions of transplant patient to a variety of services. Although the majority have a dedicated hepatology rotation (91%), the rotation experience varied from exclusively inpatient, outpatient or a combination of both, with 10% reporting this as an elective rotation. Confidence (Likert 4-5) in outpatient hepatology training did not improve from first to third year of training. Clinical confidence was highest in diagnosing and managing NAFLD and neonatal cholestasis (75.8% each). Confidence was associated with completion of a dedicated hepatology rotation, training at a liver transplant center and increased frequency of didactics Conclusions Trainee confidence in their training and their skills in diagnosis and management of liver disease is suboptimal. Confidence is directly correlated to patient exposure and hepatology focused didactic frequency. Commitment to improving hepatology education is necessary to advance training for the next generation of pediatric gastroenterologists.
