Abstract
Diagnosis of lower urinary tract (LUT) dysfunction with urodynamics has historically relied on data acquired from multiple sensors using nonphysiologically fast cystometric filling. Meanwhile, state-of-the-art treatments for bladder overactivity involving neuromodulation of LUT nerves rely on constant, open-loop stimulation because automated tools for analyzing single-sensor pressure data are not available. Using bladder pressure recordings from 14 human subjects, we have developed Context Aware Thresholding (CAT), a tunable algorithmic framework capable of real-time bladder event classification from single-sensor pressure data. We compare six event detection algorithms using a metric termed Conditional Stimulation Score, which projects stimulation efficacy and efficiency. We demonstrate that algorithms employing adaptive thresholding methods are more robust against daily variations than static thresholds, and that compared to other methods, CAT is fast, robust, noise-tolerant, highly-accurate, and amenable to energy-efficient hardware implementation. To the best of our knowledge, this is the first effort towards real-time detection of meaningful bladder events which can be used in both diagnosis and treatment of urinary incontinence.
