Clinical Evaluation and Enhancement of a Medical Case-Based Decision Support System

This thesis presents work evaluating and enhancing the Data Driven Diabetes Decision Support System (4 Diabetes Support System). Diabetes aects 23 million Americans and has severe long term consequences such as heart disease, amputation or premature death. The 4 Diabetes Support System is a prototypical case-based medical decision support system previously developed at Ohio University. It detects problems in blood glucose control and suggests solutions to these problems based on similar past cases. The system was evaluated in a clinical study involving 30 patients. Physicians veried that detected problems were correct 97.65% of the time and were useful 95.97% of the time. The cases retrieved for the problems were found to be similar 79% of the time, and the suggested solutions were judged to be benecial for the patients with the problems 82% of the time. The system was enhanced in three ways. Enhancements to reduce the time required for data entry were designed and implemented. A novel variability metric, including two new measures of variability, was proposed to measure long term improvement in blood glucose control, and ve new problem detection routines were developed. The evaluation and enhancement are important steps toward making the 4 Diabetes Support System a clinically viable system.
