Abstract
Children with special health care needs (CSHCN) are those who require additional health care services beyond that which is required by children generally. Dental care remains one of the greatest unmet health care needs in this population. CSHCN have worse health outcomes if they are not being treated in a medical home setting, and likely would have improved dental outcomes if they were treated in a dental home. The dental home has been characterized by the American Academy of Pediatric Dentistry as a dental care setting that provides accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective care. The dental home concept has not previously been validated by caregivers of CSHCN. The purpose of this study was to access caregiver prioritization of the dental home components. A cross-sectional design was used to survey caregivers of children diagnosed with cerebral palsy who were patients at Nationwide Children’s Hospital. Caregivers were asked to use a visual analog scale (VAS) to record the importance of 35 features of a dental practice, which reflected the dental home components. Data were collected from 191 caregivers. Caregivers ranked components in order of most important to least as follows: family-centered, comprehensive, continuous, coordinated, compassionate, accessible, and culturally effective. The majority of the dental home components were important to caregivers (>50% VAS), which helps to validate the dental home concept. Family-centered care was the most important component suggesting dentists should be considerate of family involvement in care for children with cerebral palsy.
