Condom Use Situations Among Chinese Men Who Have Sex With Men: Results From An Online Cross-Sectional Study

Background: Men who have sex with men (MSM) are a high-risk population for HIV infection and transmission in China. In 2015, 27.2% of new cases of HIV in China were identified among MSM. In the same year, 8% of MSM were living with HIV. The frequency of having condomless anal sex (CAS), and the reasons for engaging in the practice, remains unclear. In addition, nationwide studies have been limited by the MSM population’s mobility and stigma surrounding in China. Objective: Study objectives were to determine the prevalence of CAS in the last month, describe reasons for CAS among Chinese MSM, and determine the association of sociodemographic characteristics and high-risk sexual behaviors with the experience of having CAS in the last month among MSM in China. Methods: The analysis used data from a national cross-sectional survey, which was conducted online among Chinese MSM in 2015. The outcome of interest was the experience of having CAS in the last month. Variables with p<0.25 in the bivariate logistic regression model were included in a full multivariate logistic regression model. The final adjusted multivariate logistic regression model was created using backward selection. The statistical significance threshold was P<0.05. In addition, we also described the participants’ sociodemographic characteristics and reasons for having CAS. Results: In total, 431 MSM were recruited in the final analyses and 63.3% reported engaging in CAS at least once in the last month. The two most commonly reported reasons for having CAS with primary and casual male partners were the participant’s own preference and the availability of condoms. Multivariate analyses indicated that having multiple (AOR=2.1; 95%CI, 1.4-3.2) and casual (AOR=2.2; 95%CI, 1.4-3.4) male sex partners were associated with having CAS in the last month among Chinese MSM. Conclusion: The results indicated condom use promoting programs should target MSM who have multiple or casual male sex partners. In addition, education programs are needed to promote the benefits of using condoms and correct the misconceptions towards condom use among Chinese MSM.
