Effect of Lumbopelvic-Hip Complex Stability Training on Clinical Measures of Postural Stability and Landing Biomechanics

The lumbopelvic-hip complex (LPHC) forms the center of the kinetic chain. LPHC stability is the maintenance of pelvic and spinal alignment during perturbations and consists of neuromuscular control, muscular strength, and muscular endurance. LPHC stability deficits are related to lower extremity injury risk. LPHC stability training is shown to benefit performance and decrease risk of lower extremity injury by improving biomechanics. This study’s purpose was 1) evaluate relationships between a clinical test of LPHC stability-seated trunk control test (STCT) and other clinical tests of injury risk-anterior reach test (ART) and landing error scoring system (LESS), and 2) evaluate the effect of LPHC stability training on the STCT, ART, and LESS. We hypothesized 1) STCT performance will share a positive relationship with ART and LESS performance and 2) individuals will improve on the STCT, LESS, and ART after performing LPHC stability training. 48 uninjured male and female club sport athletes participated in this randomized control trial with secondary cross-sectional analysis. The intervention group (n=24) performed the LPHC training protocol twice weekly for 8 weeks. The control group (n=24) completed a weekly survey. Dependent variables were average number of errors on the STCT, between-limb reach distance on the ART, and average LESS scores, measured at Baseline, Week 4 and Week 8. Pearson r correlation assessed the baseline relationships between STCT performance and the ART and LESS. A 2x3 Mixed-Model ANOVA assessed between and within group comparisons. We found no significant correlations between the STCT and the ART (r= -0.093, p=0.528) or the STCT and the LESS (r=0.090, p=0.545). Group assignment and time had no effect on STCT, ART, or LESS performance. LPHC stability measured with the STCT was not related to ART and LESS performance indicating LPHC stability cannot be assumed from ART or LESS performance. Additional screening tools should evaluate LPHC stability. LPHC stability training did not improve performance on the clinical tests used in this study but could have implications for other measures of injury risk. Clinicians should not be discouraged from using LPHC based exercises in intervention.
