Abstract
Hypospadias is a birth defect of males in which the urethral meatus, is abnormally located on the ventral side of the penis. Studies have shown an association between prematurity and hypospadias, suggesting that common factors have an impact on both IUGR and hypospadias via a common mechanism of placental insufficiency. Using patients diagnosed with hypospadias, we assessed the association of prematurity with the severity of hypospadias, as well as some other risk factors. Methods: A retrospective chart review of patients diagnosed with hypospadias born either prematurely or full term was completed. Association was tested with logistic models. Results: 210 patients were analyzed. 157 were born full term and 53 were born prematurely. A marginal significant plurality and severity interaction was detected (p=0.069), suggesting that different prematurity-severity association may exist in singletons compared with multiples. In the subsequent stratified analyses, significant association was detected between prematurity and severity in singletons (p=0.020, Fisher'ss exact test). In addition, almost half of all the premature newborn males diagnosed with hypospadias also were diagnosed with a musculoskeletal congenital anomaly. Conclusions: There is a need to investigate if an underlying vascular disruption is associated with prematurity and severity of hypospadias. In addition, there is a need to explore the correlation between musculoskeletal abnormalities and hypospadias in babies born prematurely.
