Applications of Hyperpolarized 129-Xenon Magnetic Resonance Imaging in Pediatric Asthma

Background: There have been significant advances in treatment for severe asthma in the past decade, but there are no diagnostic tests sensitive to changes in regional lung ventilation in pediatric asthma. Measurements of regional lung ventilation with hyperpolarized 129Xe magnetic resonance imaging (129Xe MRI) in childhood asthma do not require ionizing radiation and are poised to advance our understanding of disease mechanisms in a highly spatially heterogeneous disease, leading to improved outcomes. Objective: The purpose of this study was to examine the relationship between 129Xe MRI, functional measures, and clinical severity in a cohort of pediatric asthma subjects. Methods: Thirty-seven children (13 severe asthma, 7 mild/moderate asthma, 17 age-matched healthy controls) aged 5-18 years old were imaged with 129Xe MRI. Ventilation defect percentage (VDP) and image reader score were calculated and compared to spirometry and clinical severity at baseline (12 months before MRI) and at follow-up (up to 12 months after MRI).
