Transition Readiness in Adolescents and Young Adults with Sickle Cell Disease

Transition planning has emerged as a standard component of pediatric care, ensuring that adolescents and young adults (AYA) have the skills and knowledge to effectively transition to the adult healthcare system. However, AYA with sickle cell disease (SCD) face worsening disease status as they advance into adulthood, coupled with low ratings of transition readiness (TR), poor self-management skills, and limited knowledge of the transition process. The present study examined the relationship between TR and chronological age, disease severity, health-related quality of life (HRQL), and disease self-efficacy in a sample of 79 AYA with SCD (ages 13-22) from an urban pediatric medical center. Results indicated that chronological age accounts for a significant proportion of the variance in TR, with young adults showing increased TR relative to adolescents. Significant differences also emerged in HRQL across disease severity groups, such that AYA with severe SCD evidenced poorer HRQL than patients with mild SCD. Study findings contribute to our growing understanding of TR and may assist healthcare providers as they aim to improve TR in AYA with SCD and identify patients at-risk for transition difficulties.
