Abstract
Optimizing outpatient care delivery for patients with iron deficiency anemia is imperative to improving the continuity of care in the primary care setting. It is appropriate for patients with iron deficiency anemia to receive their initial assessment and disease management by the primary care provider. The setting utilized for this project was found to have primary care providers bypassing an initial workup of their patients related to iron deficiency anemia and inappropriately referring them to hematology specialists for management. Reducing unnecessary referrals to hematology for patients with iron deficiency anemia can be and should be initially managed by primary care through implementation of referral protocols and care pathways for guidance. Primary care providers must thoroughly collect and record pertinent information to ensure appropriate care delivery of anemic patients. The purpose of this Doctor of Nursing Practice project was to determine if implementation of a care pathway for patients with anemia would impact the organization of care delivery and lead to a reduction of unnecessary referrals. The implementation of this project was within a large health network that supported change and adaptation of provider workflow to facilitate connected care delivery.
